SVUSD require parents wishing to help with driving students or chaperoning students overnight to complete a set of paperwork prior to being authorized to do so.

• Volunteer Form
• Scan/photocopy of your driving license
• Chaperone Form (form needed for each field trip)
• Driver Form (only needed if driving a student) (not needed for field trips with buses)
• Copy of car registration (car that you will be driving to the field trip)
• Copy of the insurance declaration page that lists limits
• TB test – you can book with own health practitioner or with a CVS minute clinic etc. Prices vary depending on your insurance.  Allow 3 days in between the test being given and the “check” you are clear.
• Live Scan Background Check (Fingerprinting) - Take the completed Live Scan form to a facility near you and submit the clearance to Julia – Costs approximately $67

 

Once you have completed these forms you should email your completed packet to julia.dunbar@simivalleyusd.org in the main office.  Once received, if you are volunteering to be an overnight chaperone she will register you to take an online training course/test.  She will email this link directly to you.
